2025-2026 Sports Physicals
ATTENTION:
Any student-athlete in grades 6-11 interested in participating in a middle school or varsity sport for the 2025-2026 school year can get a sports physical on Friday, May 16, 2025, for free at the school. This will be on a first-come, first-served basis. To be placed on this list to get your physical, you must return a completed PIAA physical form to Ms. Naccarato or our Athletic Trainer, Leigh Keller. All information must be completed on all the form pages to reserve a time slot. Please make sure you and your parents/guardians sign all the pages and all required information is provided. The completed PIAA physical form must be returned by Wednesday, May 14, 2025, by 2:00 PM to be placed on the list. Forms will also be available in Ms. Naccarato's Office or the High School Office.
